This document must be completed and returned to the school along with the child’s birth certificate, Medicare immunisation record (ACIR) and proof of address. Please see the Application for Enrolment Forms for more detailed information on the required forms.
Immunisation History Statement (ACIR)
All parents are requested to obtain a current copy of the Immunisation History Statement (ACIR).
These can be requested by:
- Logging on to your MY GOV account
- Telephoning ACIR on 1800 653 809
- Emailing acir@humanservices.gov.au
- Visiting their local Medicare branch. Parents are automatically sent an ACIR Immunisation History Statement when their child turns 18 months of age and again at five years of age (or after your child has completed the four year old vaccination schedule, whichever comes first). Please note: This information is a requirement of Department of Health and the School Education Act of 1999. Parents have the right not to have their child immunised, however parents are asked to inform the Principal in writing upon enrolment if they are conscientious objectors to immunising their child.

Kindy Enrolments 2022
Children born 1st July 2017 to 30th June 2018 can attend Kindy in 2022. Government policy introduced in July 2019 dictates that ALL students attending Kindy MUST be fully up to date with their immunisation.
Forms can be downloaded from the link above or are available from our School Administration building. An email confirmation will be sent confirming we have received your enrolment.
Information is also available from the School Office in relation to our Kindy Transition Program commencing on Wednesday, 2 November 2022.

Out of Area Students
Bibra Lake Primary School accepts Out of Area students (depending on availability of places). Our Local Intake Area children will be given priority.
All Kindy students will need to re enrol for Pre-Primary, which is their first year of compulsory schooling. Out of area children accepted into Pre-Primary will not have to re apply each year.
